Voting Application Forms

Note: Please go to the Sedgwick County Election Office website (or call 660-7100) to determine the date the forms below must be submitted for any given election.

Click here for the Federal Registration Form (in English). NOTE: Already registered but you recently moved or changed your name or want to change your party affiliation? Use this form to re-register.

In Spanish (en Espanol), we're looking for the "Aplicación para Obtener el Registro de Votación" and will post a link when we have one.

Prefer to vote by mail? Click here for Application for Advance Ballot by Mail and mail or fax it to the SG Co. Election Office. The Election Office starts mailing ballots 20 days before Election Day. The voted ballot must be returned to the Election Office by 7 p.m. on Election Day. Please note that you must submit a separate advance voting ballot application for each election unless you're applying for permanent advance voting status due to a permanent illness or disability.

Military and overseas voters can vote in federal elections by submitting to the Election Office a completed Federal Post Card Application. This allows a ballot to be mailed 45 days before the election so it can be returned by Election Day.
